Child Care Enrollment Packet

Thank you for choosing Smiles Child Care.

This enrollment packet collects all information required by Colorado child care licensing and by our program policies.

Please complete all sections honestly and completely. Incomplete packets may delay enrollment.

You will need the following items available:

All information submitted is confidential and stored securely.

Child Immunizations record or exempt form
Health Physical (Signed by a licensed medical provider) within 30 days of enrollment
Emergency contact Information
Any custody or court documents (If applicable)
Photos of your child and immediate family

Full time Child Care is $250 for four or five days a week.

Daily Care is $75 for the first day of the week, and $65 for the any additional days.

Drop-in Child Care, an extra day during the week, and overnight care is $65 a day.

A ride, pick up or drop off, is $10 each way.

Any child can participates in the once-a-month sleep over and any other activities that we do.
